Impact of Corruption on the Peruvian Health System: A Systematic Review

Abstract

Corruption in healthcare represents a structural barrier that undermines equity, efficiency, and trust in the health system. In Peru, its effects are more critical in rural areas and during emergencies. The objective of this study was to determine how corruption in the Peruvian health system influences the decision-making of public health professionals regarding patient care. A qualitative and mixed-methods systematic review was conducted using six databases (2000–2024). Study selection was based on PEO criteria. Methodological quality was assessed using ENTREQ, CASP, and JBI-QARI, while confidence in the findings was evaluated with GRADE-CERQual. The analysis was thematic and comparative. Of the 28 studies identified, three met the inclusion criteria. The findings show that structural corruption limits the implementation of technologies, generates cost overruns in procurement, affects the sustainability of projects, and reduces institutional trust. Medical professionalism was identified as an indirect protective factor, although insufficient to prevent corrupt practices. It is concluded that corruption negatively impacts healthcare delivery and health management in Peru. The implementation of transparency policies, accountability mechanisms, and citizen participation is urgently needed.
